This list is not a guarantee that your insurer will pay for your treatment. Furthermore, I may be on some panels not listed here. Please verify with your insurance company directly. You are responsible for obtaining preauthorization for your first session of your insurer requires it. Pacificare, Providence, Value Options, ODS, and United sometimes do and MHN always does.

*Many insurance panels offer out of network benefits at a lower rate of coverage so that you can see non-panel providers if you choose. I am happy to provide the necessary paperwork for you to submit for reimbursement. If you do not see your insurer listed here and/or you are not sure if you have out of network coverage, please check with your insurer directly.
Fees
***Please keep in mind that many plans have deductibles which will need to be met at the beginning of each plan year (usually the calendar year but check with your insurer). This means that your insurance will not start paying until you have paid the deductible amount out of pocket so you will be responsible for the full session fee rather than just your copay. The full fee may be less than what you see listed below as different insurance companies have different reimbursement rates. Please check with me if you need assistance determining how many sessions it will take to meet your deductible.
Payment of your copay/deductible (if you are using your in-network or out-of-network insurance benefits) or of the entire session fee (if I am not a panel provider for your insurance and you have no out-of-network coverage or you are not using insurance) is due at the time of service. I accept cash, check, Visa, Mastercard, American Express, and Discover.
